Forms
Below you will find all of the up to date forms you will need throughout your Master's or Ph.D. program. Please read through each of the forms very carefully. Forms that are not completed correctly or are incomplete will not be accepted. Also be conscious of the deadlines for each of the forms, as final signatures may take up to 24 hours to be completed. College of Engineering Forms
Doctoral Progress Report
Doctoral students must fully complete the Doctoral Progress Report form at the end of every semester. Once the form is completed, it should be turned in to the Dean's Office at the Engineering Annex Rm. 148. Once approved, you will receive an e-mail notification stating so and we will proceed with your appointment. Appointments for the following semester will not be authorized until the form is approved. Approved Doctoral Progress Reports will be kept in your file at the Dean's Office. The form is for the College of Engineering purposes only and should not be confused with the Graduate School's Doctoral Candidacy Progress report form.
Graduation Clearance Form
The Graduation Clearance Form must be submitted by every MS and PhD student upon graduation.

- Undergraduate/Graduate Dual Credit Program Application
The Undergraduate/Graduate Dual-Credit Program enables outstanding undergraduate UTEP students to receive dual undergraduate and graduate credit for up to 15 hours of UTEP graduate course work as determined by participating master's and doctoral programs. See the application for program requirements.
- Application for Graduate Certificate
This form is for students completing a graduate certificate.
- Application for Graduate Degree
This form is for students applying for graduation.Students must attach a final degree plan for further processing.
- Graduation Final Degree Plan
Submit with the graduation application.

This form must be filed by the conclusion of the first year.
- Application for Candidacy
This form must be filed after completing all qualifying exams and after defending dissertation proposal.
- Defense/Completion Form
All students must submit this form to verify successful defense of the dissertation.
- Doctoral Candidacy Progress Report
Graduate School form, not to be confused with the College of Engineering Doctoral Progress Report described above.
Master's Thesis Packet
- Degree Plan
This form must be filed by the conclusion of the first year.
- Thesis Proposal Application
This form must be filed after completing all qualifying exams and after defending dissertation proposal.
- Defense/Completion Form
All students must submit this form to verify successful defense of the thesis after applying for graduation.
Miscellaneous Forms
- Reservation for Graduate Credit
Submitted by undergraduate students requesting enrollment in graduate level course for graduate credit.
- Verification of TA/RA/AI Participant Award
Must be submitted before Census Day of each semester. Account and amount information MUST be provided for further processing.
- Library Carrel Application
Please contact the Graduate School for more information regarding the wait list, availability, etc.
Forms for International Students
- Confirmation of Financial Resources (En Español)
Before UTEP can issue your I-20 (F visa) or your DS-2019 (J visa), you must prove that you have the financial resources available to meet the minimum estimated cost of tuition, books and living expenses (if applicable). (Note: Summer attendance is optional unless you are beginning or ending a program in the summer.
- Curricular Practical Training (CPT) Application
For international students: Opportunity to participate in temporary employment directly related to their academic program/major field of study as specified by immigration regulations. Must be submitted with an offer letter from the employer and an interest letter from the students stating how the position related to the field of study. Must be undertaken PRIOR TO DEGREE COMPLETION.
- CPT Renewal Application
Must be submitted with an offer letter from the employer and an interest letter from the students stating how the position related to the field of study.
- Optional Practical Training (OPT)
OPT enables International students to accept employment after graduation. The work does not need to be for credit, but MUST BE MAJOR RELATED.
- PASE Program Application
PASE is a financial assistance program for Mexican students. This program provides its participants the opportunity to pay tuition as a Texas Resident. PASE lasts one academic year beginning in the fall and continues through the following spring and summer semesters. Each participant must reapply for the tuition discount each academic year.
- Reduced Course Load
Must be submitted prior to Census Day of each semester. This form usually only applies to students in their final semester of course study.
